IMMEDIATE OPENING FOR HOME HEALTH RN/LPN

Home Health RN/LPN- Galesburg: 8-12 hour daytime shifts

Community Care Connections has an immediate opening for an RN/LPN who is responsible for coordinating and providing patient care.

Community Care Connections is a member of the Medicalodges, Inc. family. Medicalodges offers a continuum of health care options which include in-home services, independent living, assisted living, skilled nursing home care, rehabilitation, specialized care, outpatient therapies, adult day care, as well as services and living assistance to those with developmental disabilities.

JOB RESPONSIBILITIES:

• Assessing patient care needs according to diagnosis, capabilities and plan of care.
• Coordinating patient care needs.
• Physical care of assigned patient.
• Obtaining/writing/interpreting doctor’s orders and the patient's plan of care.
• Coordinating with other disciplines and/or the Case Manager’s plan of care to meet patient needs.
• Timely submission of all required documentation.

JOB REQUIREMENTS:

• Current as a Registered Nurse or LPN in good standing in the state of Kansas
• Current CPR certification or willingness to obtain
• Strong written and verbal communication skills
• Ability to prioritize and make sound nursing decisions within scope of practice
• Possess leadership skills
• Basic computer skills
