How many IBS remedies are too many?

Dear Dr. Roach: I have dealt with irritable bowel syndrome for over 40 years. I have had less cramping and loose stools when I quit drinking milk. However, now over the past few years I have had mild constipation and very frequent bloating. I take dicyclomine, 10 mg, before meals and bedtime, that helps a little with spasms. When bloating is more constant, I take Phazyme, Tums and Pepto-Bismol, along with 150-mg ranitidine, which helps a little. I am concerned that I am taking too many types of pills to control it. I have been getting acid reflux lately and feel pressure in the stomach and up under the breast bone with a lot of burping.

Have you heard the one about the offensive joke?

Silly question: I have a neighbor who I run into a few times a week as we walk our respective dogs, and for the past six months she has made a variation of the same joke EVERY SINGLE TIME. It’s based on an aspect of my appearance that changed around then, and is not mean or insulting, but I’m over it from sheer repetition. I think it started as a legit “wow!” reaction, and the fact that I replied jokingly as if I was unaware of the change made it seem like “our schtick.”

Some cancers very difficult to catch

Dear Dr. Roach: In a recent column on screening tools, you made a brief statement about ovarian cancer. As a cancer survivor with metastasis to the ovary I was hoping to have read something within the response on screening for ovarian cancer. What is your response to someone in regard to screening for ovarian cancer if she presents with symptoms? — M.L.

Sister shows signs of mental illness at college

Dear Carolyn: My sister is 18. She got homesick at college this year and is definitely depressed. I think she recently started antidepressants. She had a brief relationship there and has reacted poorly to the breakup. She has told the guy, several times, that she’s going to kill herself. Even saying it was “in progress,” which hasn’t been true. She’s told him she’s pregnant, then says she had an abortion, then says she’s pregnant again. This has been going on for a couple of months.
